General Office Information

The Illinois Valley Center for Plastic Surgery has provided care to patients since 1994. Dr. Sheridan, a cosmetic surgeon in Peoria, recently joined the specialists at Mid Illini Surgical Associates, S.C.  to offer even more comprehensive care in the Peoria area.

Our offices are located in the Methodist Atrium, 900 Main Street, Suite 530. 

 

Consultations

Consultations are performed in the office to provide the best evaluation of the individual needs of the patient. Initial consultations are scheduled 45 minutes to one hour. Contact us for additional information or to schedule your appointment to discuss your options.

 

Before Your Appointment

Please arrive 15 minutes prior to your scheduled appointment to complete the necessary patient information forms.

We are open from 8:00 a.m. to 4:30 p.m., Monday-Friday. To schedule a consultation, please call (309) 691-7800.

 

Insurance & Billing

Illinois Valley Center for Plastic Surgery participates in most major insurance programs and will bill your insurance carrier on your behalf. As an additional service to you, we will assist you with completing prior authorization and pre-certification with your insurance carrier during your visit. You are responsible for your yearly deductible, co-payment and any balance not covered by your insurance company.
